This survey is conducted by Helios Clinical to understand public interest in learning about the risks of cardiovascular disease (CVD) related to Lipoprotein(a), or Lp(a), levels, especially in individuals with a family history of CVD. Your responses will help guide the development of future clinical research trials. All responses are confidential.

Terms and Conditions

Open to legal residents of Arizona, USA, aged 18 years or older. Employees of Helios Clinical Research, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible to participate. To enter the giveaway, participants must complete the survey in full. Incomplete surveys will not be considered for the giveaway. *One winner will receive one home blood pressure monitor valued at $~30.00. The specific brand and type of blood pressure monitor will be determined by Helios Clinical Research based on availability. The giveaway begins on 5/21/2024 and ends on 6/30/2024. All survey responses must be received by 6/30/2024 to be eligible. The winner will be selected at random from all eligible entries received during the entry period. The odds of winning depend on the number of eligible entries received. The winner will be notified via email/phone within 5 business days of the giveaway end date. If the winner does not respond within 5 business days, an alternate winner will be selected. Helios Clinical Research reserves the right to cancel, suspend, and/or modify the giveaway, or any part of it, if any fraud, technical failures, or any other factor beyond Helios Clinical Research’s reasonable control impairs the integrity or proper functioning of the giveaway, as determined by Helios Clinical Research in its sole discretion.
