To complete your enrollment, Helios Clinical Research uses a secure health record service called Pluto Health. The attached Pluto ROI (Release of Information) and HIPAA Authorization Form allow Helios, through Pluto Health, to request and review your relevant medical records directly from your healthcare providers. This ensures that we have accurate and up-to-date information to confirm your eligibility for the study and coordinate your care appropriately.

By signing these forms, you are authorizing:

• Helios Clinical Research to share your registration details with Pluto Health so your records can be gathered on your behalf.
• Pluto Health to securely obtain your protected health information (PHI), such as lab results, diagnoses, and treatment history relevant to the study, and share it only with authorized study personnel involved in your care.

In order to get started, we need you to do the following:

• Review the attached forms carefully.
• Sign where indicated.
• Return them to us at your earliest convenience by emailing Cerritos-Screening@heliosclinical.com.
